Alerjik Rinitte Epikütan MuItitest'in Spesifik IgE Düzeyine Göre TanısaI Değeri*Döner F., Doğru H., Tüz M., Uygur K., Turgut B.Amaç: Alerjik rinitli (AR) olgularda alerjene spesifik duyarlılığın belirlenmesinde epikutan Multitest'in spesifik 19 E düzeylerine göre duyarlılığı ve özgüllüğü araştırıldı.
Hastalar ve Yöntemler: Burun akıntısı, kaşıntısı, hapşırma ve göz yaşarması gibi şikayetlerle başvuran, anamnez ve fizik muayeneleri sonucunda AR tanısı konan 27 olgu (10 erkek, 17 kadın; ort. yaş 30) çalışmaya alındı. Tüm olgularda nazal sekresyonlar ve periferik kanda eozinofili değerlendirildi. Multitest yöntemi ile cilt testleri yapıldı. Serum total IgE "Cobascore Otomatize MakroELISA", spesifik IgE ELISA yöntemiyle ölçüldü.
Bulgular: En sık görülen semptomlar burun tıkanıklığı ve hapşırma idi. Epikütan Multitest'in spesifik IgE düzeyine göre duyarlılığı, incelenen alerjen gruplarında %70.0 ile %83.3 arasında bulundu. En düşük ve en yüksek duyarlılık oranları sırasıyla C1 A ve D2 gruplarında saptandı. Multitest'in özgüllüğü ise %71.0-%90.2 arasında bulundu. En düşük özgüllük oranı C1A grubunda, en yüksek C1 B grubunda saptandı.
Sonuç: Epikütan Multitest'in AR 'de alerjen saptanmasında kısa sürede sonuç alınabilen, ucuz, çocuklara bile kolay uygulanabilen, duyarlılık ve özgüllüğü en yüksek cilt testlerinden biri olduğu görüldü.
Diagnostic Value of Epicutaneous Multi-test Regarding to The Level of Specific IgE in Allergic RhinitisDöner F., Doğru H., Tüz M., Uygur K., Turgut B.Objectives: The specificity and sensitivity of epicutaneous skin test (Multi-Test"') were assessed, in comparison with specific IgE levels, in determining specific sensitivities to allergens.
Patients and Methods: The study included 27 patients (10 males, 17 females; mean age 30 years) with allergic rhinitis (AR). All the patients had complaints of rhinorrhea, nasal itching, sneezing, and epiphora. In all cases, eosinophils were evaluated in nasal secretions and in peripheral blood. Skin tests were pertormed using the Multitest. Total lgE levels were measured by the "Cobascore Otomatizeo MacroELISA" method and specific IgE levels by the ELlSA method.
Results: The most common symptoms were nasal obstruction and sneezing. The sensitivity and specificity of Multitest for specific IgE ranged from 70.0% to 83.3% and from 71.0% to 90.2%, respectively, in the allergen groups studied. The lowest and the highest sensitivity rates were found in the C1 A and D2 groups; the lowest and the highest specificity rates in the C1A and C1B groups, respectively.
Concluslon: Epicutaneous Multitest proved to be easy, cost-effective, readily applicable, and highly sensitiye and specific for allergens in AR patients.
