A Modification of Nasal Septal Dermoplasty in Hereditary Hemorrhagic Telangiectasia*Özcan C., Ünal M., Görür K.Herediter hemorajik telanjiektazi, deri ve mukozat yüzeylerde kolayca kanayan telanjiektazilere yol açan, otozomal dominant bir kalıtsal bozukluktur. Otuz yedi yaşında erkek hasta, belirgin bir aile öyküsü ve 25 yıldan fazla bir zamandır tekrarlayan burun kanamaları ile başvurdu. Nazal septumu ortaya koymak amacıyla transkolumellar-transkrural modifiye eksternal rinoplasti yaklaşımı uygulandı. Kasıktan 20x25 cm'lik bir splitthickness deri grafii alındı. Septal mukoza iki taraflı kaldırıldıktan sonra, tek parça olarak deri grafii öndeki insizyondan 'U' şeklinde geçirildi. Önce graftın dezepitelize edilen orta kısmı vestibül cildine dikildi. Daha sonra greft septuma trans septal matris dikişler ve septal splintle yerleştirildi. Greftin tutulumu tam ve erken fonksiyonel sonucu başarılı olarak değerlendirildi.
Herediter Hemorajik Telanjiektazide Nazal Septal Dermoplasti Tekniğinin ModifikasyonuÖzcan C., Ünal M., Görür K.Hereditary hemorrhagic telangiectasia is a hereditary disorder with autosomal dominant trait, leading to bleeding on the skin and mucosal surfaces. A thirty-seven-year old mala patient presented with an obvious history of repeated episodes of epistaxis for more than 25 years. A modified transcolumellar-transcrural external rhinoplasty approach was employed to expose the nasal septum. A split-thickness skin grafi, 20x5 cm in size, was obtained from the groin. After the bilateral elevation of the septal mucosa, the single-skin graft was passed through the anterior incision in a U-shaped pattern. First, the desepitelized midportion of the graft was sutured to the vestibule skin. Then the graft was fixed to the septum by transseptal mattress sutures and septal splints. The graft incorporation was complete and early functional results were successful.
