Background; Pectoral’s major flap the best flap for cases that there is wide defect with low blood supply so that risk for peripheral flap failure is very high. So there is doing internal pectoral’s major with problems, including balking – cosmetic – distance and failure blood supply because tension on base of flap pedicle …. Case presentation; the patient is man with 56 years old that from child hood follow trauma to face have face defect, and after this state he is try for Repair but fail that he is refer to this center with large defect on face.becase he did failure for deltoid flap before so I have planning for muscular flap with skin And muscle for coverage of defect and insurance of blood supply. Results; external pectoral’s major flap pedicle cut after 3 week with good flap supply. Conclusion; external pectoral’s major flap is new technique for cases that distances or baulking problematic.
